Standard Additional Term Life:
Employees may purchase additional term
life insurance either at the time of hire or during
the annual Open Enrollment period. Evidence of insurability
is required for any amount above the $10,000 the City
furnishes to you at no cost. Employees may purchase
up to an additional $100,000 worth of term life. All
coverage ceases if the employee terminates his or her
employment with the City.
Qualified Retirees are covered for
the basic $3,000 at no cost. Additional term life insurance
may be purchased at the time of retirement IF
they were purchasing $10,000 to $20,000 while employed.

Standard Dependent Term Life: With the basic
$10,000 at no cost to employee, you can purchase $5,000
of dependent term life for a spouse and $2,500 for each
child for a deduction of $0.48 per week. If you purchase
a minimum of $20,000 on yourself, the Dependent Life
insurance can be increased to $10,000 for a spouse and
$5,000 for each child. Deduction for this coverage is
$1.05 per week. *Child means your unmarried child from
live birth through age 20 (through age 24 if a registered
student in full-time attendance at an accrediated educational
institution).

FBMC/Flexible
Medical Spending Account: The Federal Government
allows employees to have monies deducted pre-tax to be
used toward many medical out-of-pocket expenses. As this
program is federally regulated, care must be taken to
accurately estimate expenses. Minimum deduction is $2.00/wk
($104.00/yr). Maximum deduction is $96.15/wk ($5,000/yr).
You can also be reimbursed for many Over The Counter (OTC)
medicines and supplies starting January 1, 2004. Further

Open Enrollment.

Flexible
Dependent Care Spending Account: Similar to the
Medical Spending Account, monies are deducted pre-tax
for costs related to dependent care. On this plan your
claim will not be reimbursed until funds are available
in your account. This option is available only during
Open Enrollment.

OptiCare
Vision Plan: For employees
the cost is $1.45/wk for single coverage or $3.49/wk
for family coverage. For retirees the
cost is $6.29/mo for single coverage or $15.12 for family
coverage. This plan includes one routine vision exam
and new lenses every 12 months; frames every 24 months;
$125.00 allowance for contact lenses.
Exam
co-pay is $10.00
Materials co-pay is $20.00

(Legal Plan): Ultimate Advisor Plan. You and your
family are covered for $3.98/wk for Employees
and $17.24/mo for Retirees. Examples
of services covered include: Will preparation, identity
theft, traffic tickets, document review, financial and
tax planning services, real estate issues and much more.

Trustmark/Universal
Life Insurance: A whole life policy may be purchased
through the City's insurance deduction plan, but unlike
term life, the policy can be retained if employment with
the City terminates. Money can also be borrowed against
this policy. Full life policies must be purchased through
a licensed insurance agent. This option is available
only during Open Enrollment.

Trustmark/Disability:
Employees can protect themselves against disability. Based
on age, health, occupation and annual wages, this program
must also be purchased through a licensed insurance agent.

Trustmark/Cancer
Insurance & Critical Illness: This plan provides
a lump sum cash benefit upon first diagnosis of internal
cancer. Underwritten by Trustmark, this plan provides
an immediate cash benefit (up to $50,000) payable directly
to the employee. The plan is designed to increase the
treatment and lifestyle options available to covered employees
or family members diagnosed with cancer. Critical Illness
Policy is available as a rider to the Cancer Policy, or
you may opt for this policy alone. Critical Illness covers
cardiac, diabetes, for example. Call 1-800-918-8877 for

Employee
Assistance Program (EAP)

A
free, confidential counseling program offered through
Sarasota Memorial Hospital to all employees
and their families and to all retirees
and their covered dependents is the Employee Assistance
Program. Stress and worries, whether job related or not,
can affect our well being and job performance. This program
is offered to reduce the effects of these stresses with
counseling on numerous issues. The following are some
examples:

Fully
trained and certified counselors are available to assist
during those times when help is needed. You are entitled
to four visits at no cost if you wich to see a counselor
with EAP. If you wish to be referred to a network doctor,
then you will have either a co-pay or deductible and coinsurance,
depending on your medical plan. For any type of therapy
or substance abuse, you MUST go
through the EAP Program. A brochure detailing these services
is available in the Human Resources Department. If you
