An HMO-type plan offered to all employees. Basic Plan is governed
by Sarasota Memorial Hospital Physician Hospital Organization.
Basic is available to employees and dependents with doctors
in Sarasota County only.
A
Primary Care Physician (PCP) MUST be selected.
Referrals are needed for specialist care. You must choose
providers from the network or you will be responsible for
the bill. Basic providers can be accessed at www.gulfcoastprovider.net
or call (941) 917-8500.
The
Basic Plan is a blend of co-payment and deductible/coinsurance.

$15.00 co-pay for Primary Care Physician visits
$25.00 co-pay for Specialists
(SMH) Deductible and 80/20 coinsurance Lab work & X-Rays
(SMH) Deductible and 80/20 coinsurance Inpatient Hospital
(SMH) Deductible and 80/20 coinsurance Outpatient Surgery
(SMH) Deductible and 90/10 coinsurance for emergency room
(urgent care)
Wellness Benefit First $400.00 of wellness claims paid at
100% then 80/20%
You
will not be allowed to go outside the network unless there
is a need for emergency treatment.
